We have produced this website to provide you with information about the way the Practice is organised and the services that are provided directly by Practice Staff, or by other members of the Primary Health Care Team. Services will, by necessity, change from time to time, please do not hesitate to telephone the Practice to check the current position. Prime Ministers Challenge Fund We are taking part in a pilot scheme under the Prime Minister's Challenge Fund which offers our patients the opportunity to see a GP or nurse for a routine appointment when they need to. There is a network of GP practices in Shropshire which are offering extended opening hours for routine appointments. Emergency medical care when Portcullis Surgery is closed is still provided by Shropdoc. The GP or nurse whom you will see, will have access to your medical records under special data sharing agreements and you will be asked for your consent for them to access your records when making an appointment and at the start of the consultation. Your usual GP here at Portcullis will be able to see the consultation on your records. These appointments can NOT be booked via Portcullis Surgery, instead you must ring 0333 222 6649 to book or cancel one. Please note there are no appointments under this scheme available at Portcullis and the appointments you will be offered will be at various practices across Shropshire and at various times. New patients registering with the Practice will be asked to complete a Health Questionnaire which enables the Practice to manage ongoing medical conditions. Click to download our New Patient Registration Pack as pdf format(print and complete) You will be required to provide identification, e.g. Passport, Photo Driving Licence. Accountable / Named GP From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP. This is an administrative exercise in order that patients can have a named responsible GP should they require them. Your named accountable GP will be the named doctor you are registered with. If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery. Please note: there is no need to telephone the practice for this information. If you need input from a variety of health professionals, then you can discuss these with the named GP if you wish. Alternatively you can discuss these with any of the other doctors. Those of you whom receive regular repeat medications will have your named GP printed on the white repeat medication slip. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor. Temporary Residents We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms. Teaching and Research As a teaching practice, medical students spend part of their training with us from Keele University. The Practice is also involved with medical research in partnership with Primary Care Reasearch at Keele University. We would value your co-operation with both of these, but we understand if you do not want to be involved. Zero Tolerance This Practice supports the Government’s NHS zero tolerance zone campaign. GPs and their Staff have the right to care for others without fear of being attacked or abused. We ask that you treat your GP and Practice Staff with courtesy and consideration.
